This patch adds support for automatic date and time formatting based on a
selection of locale. The locale can default to the system or user specified
language+locale, or can be elected separately. For instance, English
speakers can pick between US, GB, and many other English speaking locales.

This also removes the need of the %timezone table and uses the timezonedb
built into PHP 5.3+. User's can now select from a much longer list of
database and no longer need to deal with the DST checkbox.

Move the attachments to the sidebar, add the concept of language-specific
attachments. Add a sidebar to the front page. De-emphasize the "open new
ticket" button. Add a knowledge base search feature to front page and also
add the concept of "featured" articles and categories on the front page.

This will help if the user has "Browser Preference" selected as the
preferred language. When email correspondence needs to be sent to the user,
the browser_lang extra attribute can be used as a backup to to the user's
language preference in the event that the language preference is set to auto
(Browser Preference).

This patch also makes a slight edit to mPDF to allow better detection of
substitions in characters, as the DejaVu fonts shipped do not support Thai
glyphs.

If there is a problem installing some of the items with the system, try not
to crash simply because the error is passed into another ::create()
function.

The software defines at least three contexts for the phrase "Open"

  * The name of a ticket queue (Open tickets)
  * A status of a ticket (Open / Closed)
  * An action to be performed (Open a ticket)
