This patch adds support for automatic date and time formatting based on a selection of locale. The locale can default to the system or user specified language+locale, or can be elected separately. For instance, English speakers can pick between US, GB, and many other English speaking locales. This also removes the need of the %timezone table and uses the timezonedb built into PHP 5.3+. User's can now select from a much longer list of database and no longer need to deal with the DST checkbox.

Ticket numbers are now configurable as a system-default and by help topic. Previously, an administrator could select between six-digit random and sequential numbers. Now, the sequence is selectable so multiple sequences as needed, and a format can be specified to define how the ticket numbers look. Formats can also be defined for each help topic.

This patch introduces an automatic materialized view to speed database performance when querying and displaying the ticket views. This can eventually be extended to the search and advanced search features to speed them as well. The data from the dynamic form entries related to ticket details is copied to a %ticket__cdata table. The %ticket__cdata table is then joined directly to the other tables in the query for the ticket view. MySQL is magically and dramatically faster using this method. The downside is that the disk usage for the custom data is doubled, and the time needed to update the dynamic data is at least doubled as the form entries and the materialized view must both be updated. This method should also extend well to other database platforms in the future. It will be likely that most other database query optimizers will have difficulty joining, scanning, and sorting the table models we have for custom data fields.

If an email arrives in the system without a "personal" field (name), the email address is used as the name. The email address, however, is not parsed well by the PersonsName constructor. This patch uses the mailbox of the email address as the name. Also include a slight boost to the mbstring compatibility functions declared in the bootstrap::i18n_prep() function

Previously, form sections were grouped into form sets for reusability. This patch drops the form sets and makes form sections the new "forms". Eventually a section-header field will be added that technically does not add any dynamic data to the form, but allows for the same feature as having a form set with multiple sections.
