* Copy config var name_format to agent_name_format and client_name_format
  and delete it

This stems from a bug not clearing the org_id when removing organizations

Also make forms deleteble by marking them as deleted and hiding them from
the form listing. This ensures that custom data always has a link back to
the form.

These fields have been consolidated into the `flags` column.

Few UI tweaks and Team::getLocalName

* Support @border and @cellpadding attributes
* Support padding for display:block elements

.. rather than leading. This changes the block folding logic a bit to always
follow a block with a newline rather than lead with a newline. It also
forces a leading newline when a block element follows an inline element.

Also remove `*` chars from <b> and <strong> elements and change <hr> to use
box drawing Unicode chars.

This patch removes several of the internal notes posted to a ticket for
various events and replaces them with events added to a threads event chain.
The emails are still sent out as they previously were, but internal notes
are only added to the thread if an agent actually specified comments.

Move collaboration implementation to thread level

Also, fix the links to the ticket queue search (again)
